Addiction Medicine Counselor

Due to our sustained growth, we are expanding our team of Therapists within the Addiction Medicine Program. As a Therapist specializing in Mental Health & Addiction, you will play a crucial role in a collaborative, interdisciplinary team focused on providing integrated services and programs to our clients.
Your Responsibilities:
- Conduct assessments to inform treatment planning and guide client care.
- Deliver case management services, including planning, coordination, and progress evaluation.
- Facilitate individual and group psychotherapy using evidence-based practices such as Motivational Interviewing, Trauma-Informed Care, and Cognitive Behavioral Therapy.
- Support family members through individual and group sessions, providing education and counseling.
- Collaborate closely with Physicians and the clinical team to monitor patient experiences and adjust treatment plans as necessary.
- Lead discharge planning and connect clients with appropriate community resources.
- Establish treatment goals in partnership with clients and team members, developing actionable steps to achieve these objectives.
- Oversee the evaluation of treatment progress and ensure effective communication among all stakeholders.
- Maintain comprehensive documentation in accordance with organizational policies.
Qualifications:
- Bachelor's or Master's degree in a recognized Social Science program with specialized education in addiction.
- Membership in good standing with a relevant regulatory body in Ontario or in pursuit of necessary credentials.
- Proficiency in evidence-based approaches for treating complex mental health conditions and substance use disorders.
- Demonstrated experience in assessment and treatment using evidence-based methodologies.
- Strong communication and interpersonal skills, with the ability to lead interdisciplinary teams.
- Experience in delivering inclusive and culturally responsive care.
- Knowledge of relevant legislation and policies related to mental health and addiction.
