About the Role

We are seeking a skilled Licensed Practical Nurse to join our team at Westford Health Nursing Care Inc. as a Geriatric Care Specialist. In this role, you will provide high-quality nursing care to patients in a fast-paced environment.

Key Responsibilities
  • Assess and Monitor Patients

Conduct thorough assessments of patients' physical and emotional needs, and develop individualized care plans to address their unique requirements.

Provide Nursing Services

Deliver nursing care to patients, including administering medications, performing medical procedures, and providing education on health and wellness.

Collaborate with Healthcare Team

Work closely with physicians, therapists, and other healthcare professionals to ensure comprehensive care and effective communication.

Document Patient Progress

Maintain accurate and detailed records of patient interactions, treatments, and outcomes.

Requirements
  • Education and Licensure

Eligible for licensure as a practical nurse by the province/territory of work.

Certifications and Training

CPR Certificate and First Aid Certificate.

Work Environment

Fast-paced environment with a combination of sitting, standing, and walking.

Physical Demands

Physically demanding with repetitive tasks and standing for extended periods.
