Join Vancouver Coastal Health (VCH) as a Case Manager

Vancouver Coastal Health is seeking a Registered Nurse to be part of the Long Term Care team. This role involves working closely with clients to ensure they receive the best possible care and support tailored to their individual needs.

Responsibilities of the Case Manager include:

Overseeing a designated group of Home Health clients who require specialized case management expertise. Identifying client objectives and requirements, evaluating their circumstances, collaborating with community resources, and planning care in alignment with their health needs. Ensuring seamless care throughout the service continuum by assessing clients, coordinating with team members, implementing clinical strategies, and evaluating outcomes based on client-centered goals. Serving as a clinical resource for the interdisciplinary team by sharing knowledge based on specialized nursing theory and practice for clients with intricate health care requirements. Engaging in consultation and collaboration with multidisciplinary team members, healthcare professionals, and educational resources to enhance client-centered education and research.

Required Qualifications

Education & Experience

Current registration as a Registered Nurse with the British Columbia College of Nurses and Midwives (BCCNM). A minimum of three (3) years of recent clinical nursing experience in Home Health and case management, or an equivalent combination of education and experience. Valid BC Driver's License and the ability to travel locally using a personal vehicle.

Knowledge & Skills

In-depth understanding and proven ability to apply case management principles effectively. Extensive knowledge of case management practices within a client and family-centered care framework for individuals with complex health care needs. Thorough knowledge of BCCNM standards of practice and clinical guidelines. Comprehensive understanding of care strategies for clients requiring supportive case management. Familiarity with evidence-based nursing practices relevant to supportive case management. Adherence to established visit standards for case management. Awareness of various health disciplines and their roles in client care. Knowledge of adult education principles and methodologies. Understanding of research processes and methodologies. Ability to apply critical thinking and sound judgment in problem-solving within the scope of client care. Proven leadership capabilities and the ability to guide and direct others. Proficiency in teaching, demonstrating, and facilitating care for clients and their families. Competence in developing and executing project action plans to achieve objectives. Ability to work independently and collaboratively within an interdisciplinary team. Strong oral and written communication skills. Capability to effectively interact with co-workers, healthcare professionals, clients, and their families. Skills in conflict resolution and adaptability to changing circumstances. Competence in planning, organizing, and prioritizing work collaboratively. Proficiency in conducting comprehensive assessments of physical, mental, and psychosocial needs. Ability to utilize supplies and materials effectively in community-based nursing services. Physical capability to perform the duties required of the position. Skills in techniques relevant to the treatment of the client population. Basic computer skills for operating patient care information systems. Ability to operate necessary equipment.

Work hours, including days off and work areas, may vary based on operational needs and applicable agreements. Successful candidates may need to undergo a Criminal Records Review Check.
