About the Role:
We are seeking a skilled Registered Practical Nurse (RPN) to join our team in providing high-quality community care to patients in various settings. As an RPN, you will work independently to assess patient needs, develop care plans, and provide compassionate care to patients of all ages.
Key Responsibilities:
- Participate in scheduled visits to assess patient needs and provide care within your scope of practice
- Provide safe and ethical nursing care to patients
- Use assessment tools and techniques to identify patient needs and develop care plans
- Monitor and adjust care plans as required
- Engage patients in their care through a collaborative approach
- Maintain accurate and up-to-date patient records
Requirements:
- Registration with the CNO as a Registered Practical Nurse in good standing
- Valid driver's license and access to a vehicle
- Experience in delivering community care is an asset
- Reliability and flexibility
- Ability to travel to and from patient residences within a designated area
- Experience working with vulnerable populations
- Passion for making a difference in the community
