About the Role
CarePartners is seeking a skilled and compassionate Nurse to join our team of healthcare professionals. As a Community Nurse, you will have the opportunity to work in a variety of settings, including in-home, clinic, retirement home, and school, providing exceptional nursing services to patients in need.
Key Responsibilities
- Develop and maintain effective care plans for patients receiving community health services
- Provide nursing services to patients in a variety of settings, including in-home and virtual care
- Communicate and liaise with patients, family members, and other healthcare professionals to ensure seamless care
- Perform other clinical duties as required

Requirements
- Registered Nurse (RN) degree or (RPN) diploma
- Current College of Nurses of Ontario (CNO) registration in good standing
- Strong desire and commitment to making a difference in the lives of our patients
- Exceptional interpersonal skills
- Valid driver's license and vehicle
- Excellent English verbal and written communication skills
- CPR Certification
- Current immunization record including 2 step TB test
- Vulnerable sector check and clear background check
