About CarePartners

We deliver comprehensive nursing, personal support, therapy, and rehabilitation services to patients of all ages, serving approximately 72,000 patients annually through our 24 locations and 22 nursing clinics.

Job Overview

We are looking for a highly motivated and experienced Occupational Therapist to provide high-quality patient care, collaborate with Community Support Workers, and develop effective Care Plans.

Key Responsibilities:
  • Conduct professional assessments of patients in the community and in their homes
  • Develop and maintain effective Care Plans in collaboration with Community Health Services
  • Work with patients to establish time-specific, measurable goals and outcomes in collaboration with the care team
  • Provide guidance on workplace Health and Safety
  • Perform other duties as required
Requirements:
  • Master's Degree (or equivalent) in Occupational Therapy from a recognized university
  • Registration with the College of Occupational Therapists of Ontario
  • Proven computer skills, including email, MS Word, and Outlook, work scheduling systems
  • Strong assessment skills and the ability to participate in effective treatment programs
  • Excellent English verbal and written communication skills
  • French language skills are an asset
  • Clear Background and Vulnerable Sector Check
