Position Overview

The Licensed Practical Nurse (LPN) is a certified healthcare professional dedicated to delivering high-quality, patient-centered nursing care to individuals with less complex health needs, who exhibit predictable outcomes and are at a lower risk for adverse effects in hemodialysis settings. As a vital member of the interdisciplinary healthcare team, the LPN will adhere to the nursing practice standards established by William Osler Health System, the regulatory guidelines of the College of Nurses, and the legislative requirements outlined by the Regulated Health Professions Act.

Key Responsibilities:
  • Ensure safe nursing practices throughout all phases of the nursing process: assessment, planning, implementation, and evaluation of patient care.
  • Deliver professional, patient-focused care to promote safety and enhance both physiological and psychological well-being.
  • Provide compassionate care within a culturally diverse environment.
  • Adhere to infection control and prevention protocols.
  • Respond effectively to emergencies by recognizing changes in patients' health status and consulting appropriately, utilizing transfer of accountability principles.
  • Educate patients and their families, including comprehensive discharge instructions to facilitate continuity of care in the community.
Qualifications:
  • A minimum of two years of hemodialysis experience within the last five years is required.
  • Nephrology certification is required or candidates must be enrolled and complete the Nephrology certification within 18 months of hire.
  • Completion of vascular access modules from the Ontario Renal Network is mandatory.
  • Passing a nephrology quiz with a minimum score of 80% is necessary to proceed to the interview stage.
  • Familiarity with the Nephrology Nursing Standard and Practice Recommendations, as well as the Canadian Association of Nephrology Nurses and Technologists, is essential for hemodialysis procedures, including patient preparation, treatment, and monitoring.
Core Competencies:
  • Current registration and good standing with the College of Nurses of Ontario.
  • Strong understanding of nursing processes, theories, and responsibilities.
  • BCLS certification is required.
  • Medication Administration certification and completion of a patient assessment course are required for LPNs who graduated prior to 2005.
  • Exemplary interpersonal, communication, problem-solving, conflict-resolution, and decision-making skills.
  • Ability to collaborate effectively as part of an interdisciplinary team.
  • Knowledge of patient and family-centered care principles.
  • Demonstrated leadership and facilitation abilities.
  • Exceptional customer service, organizational, and critical-thinking skills.
  • Commitment to ongoing learning and professional development relevant to the field.

Additional Information:

Work hours may include days and evenings, with shifts of 8 hours, including weekends and statutory holidays, subject to operational requirements.

Salary range: Minimum of $33.26 per hour to a maximum of $35.97 per hour.
