Job Summary

We are seeking a highly skilled Registered Practical Nurse (RPN) to join our Hemodialysis team at the William Osler Health System. As an RPN, you will be responsible for providing quality patient-focused nursing care to patients who are identified as less complex and demonstrate more predictable outcomes in hemodialysis.

Key Responsibilities
  • Safe Nursing Practice: Ensure safe nursing practice in all aspects of the nursing process, including assessment, planning, implementation, and evaluation of patient care.
  • Professional Patient-Centered Care: Provide professional, patient-centered care to ensure patient safety and physiological and psychological well-being.
  • Cultural Competence: Provide care in a compassionate and caring manner within a diverse cultural environment.
  • Infection Control: Practice according to infection control and prevention measures.
  • Emergency Response: Respond to emergency situations by recognizing changes in patient's health status and consulting appropriately when necessary using transfer of accountability principles.
  • Health Teaching: Provide health teaching to patients and families, including thorough discharge instructions to ensure the continuity of care in the community.
Qualifications
  • Experience: Minimum two years of hemodialysis experience within the last five years required.
  • Certification: Nephrology certificate required or enrolled and will successfully complete Nephrology certificate within 18 months of hire.
  • Modules: Completion of vascular access modules from the Ontario Renal Network required.
  • Knowledge: Must pass nephrology quiz by obtaining 80% to move onto interview phase.
  • Standards and Practice: Knowledge of the Nephrology Nursing Standard and Practice Recommendation and Canadian Association of Nephrology Nurses and Technologists as they relate to hemodialysis, including all procedures pertaining to patient preparation, treatment, and monitoring during and after procedure required.
Core Competencies
  • Registration: Current registration, and in good standing, with the College of Nurses of Ontario.
  • Nursing Knowledge: Sound knowledge of nursing process, theory, and responsibilities to the nursing profession.
  • CPR Certification: BCLS required.
  • Medication Administration: Medication Administration certificate and patient assessment course required for RPNs graduating prior to 2005.
  • Interpersonal Skills: Demonstrated excellent interpersonal, communication, problem-solving, conflict-resolution, and decision-making skills required.
  • Teamwork: Demonstrated ability to work collaboratively as a member of an inter-professional team.
  • Patient-Centered Care: Demonstrated knowledge of patient and family-centered care.
  • Leadership: Demonstrated leadership and facilitation skills required.
  • Customer Service: Demonstrated excellent customer service, organization, and critical-thinking skills required.
  • Continuous Learning: Demonstrated commitment to continuous learning and professional development as it applies to the profession and related field.
  • Stress Management: Demonstrated ability to work in a stressful environment.
  • Clinical Practice: Exemplary clinical practice within an inter-professional team to develop appropriate strategies for patient care.
