The Licensed Practical Nurse (LPN) plays a crucial role in delivering exceptional patient-centered nursing care within the Acute Care for the Elderly (ACE) Unit. As a vital member of the interdisciplinary health care team, the LPN is responsible for adhering to the nursing practice standards established by the organization, the regulatory guidelines of the nursing college, and the legal requirements outlined by health profession legislation.
Care Model Implementation
The ACE Unit employs a patient-focused care approach known as the Butterfly model. This innovative, emotion-driven care strategy emphasizes meaningful interactions between patients and nurses, aiming to alleviate behaviors associated with dementia and delirium. The model's core principles focus on creating a supportive clinical environment that enhances cognitive, emotional, psychological, and physiological well-being for older adults.
Key Responsibilities
- Collaborate effectively within the Butterfly model team to establish, assess, and maintain the essential elements of the care model.
- Engage in educational opportunities related to the Butterfly model to reinforce its principles.
- Motivate and guide colleagues in the successful application of the Butterfly model through exemplary role modeling and effective communication.
- Participate in change management initiatives to support the transition to an emotion-focused care approach.
- Utilize emotional intelligence to develop innovative, person-centered strategies that promote the success and emotional well-being of patients living with dementia.
- Ensure safe nursing practices throughout all phases of patient care, including assessment, planning, implementation, and evaluation.
- Adhere to established policies and procedures for patient observations.
- Deliver professional, patient-centered care that prioritizes safety and well-being, fostering therapeutic relationships.
- Provide compassionate care within a culturally diverse environment.
- Follow infection control protocols diligently.
- Respond effectively to emergencies by recognizing changes in patient health status and consulting appropriately.
- Educate patients and families on health matters, including comprehensive discharge instructions to facilitate continuity of care.
Qualifications
- Minimum of two years of experience in acute medicine within the last five years.
- Experience in caring for geriatric and dementia patients in an acute care setting within the last two years.
- Strong understanding of geriatric syndromes, delirium, and dementia.
- Enrollment in a geriatric certificate program is required.
- Gentle persuasion skills are essential.
- Preferred enrollment in PIECES and NICHE geriatric certificate programs.
- A genuine passion for leading dementia care initiatives.
- Current knowledge and skills in evidence-based dementia care practices.
- Prioritize patient well-being over task completion.
- Recognize the significance of emotions in patient care.
- Experience in managing patients with TPN, NG tubes, chest tubes, tracheostomy, wound care, enteral feeds, IV therapy, blood collection, and isolation.
Core Competencies
- Active registration and good standing with the nursing regulatory body.
- Comprehensive knowledge of nursing processes, theories, and responsibilities.
- BCLS certification is required.
- Medication administration certification and patient assessment course completion required for RPNs graduating prior to 2005.

- Understanding of patient and family-centered care principles.

Additional Information
Hours: Day/evening shifts, 8 hours including weekends and holidays (subject to operational needs).
Salary: Minimum $34.26 per hour, Maximum $37.05 per hour.
