We are seeking a highly skilled and experienced Nurse Practitioner to join our Complex Malignant Hematology Team at the University Health Network. As a key member of our team, you will provide expert care to patients with complex malignant hematology conditions, working collaboratively with our multidisciplinary team to deliver high-quality patient care.
Key Responsibilities- Provide comprehensive patient assessments, develop and implement individualized care plans, and coordinate care with other healthcare professionals.
- Manage patient care in various settings, including inpatient and outpatient units, clinics, and community settings.
- Collaborate with our interprofessional team to provide education, support, and guidance to patients and their families.
- Participate in research, education, and quality improvement initiatives to advance the field of hematology and oncology.
- Develop and maintain strong relationships with patients, families, and healthcare professionals to ensure seamless care transitions and optimal patient outcomes.
- Current College of Nurses of Ontario Registration in the Extended Class (EC) as a Nurse Practitioner.
- Completion of a Master's degree in a relevant field.
- Demonstrated expertise as a Nurse Practitioner, with a minimum of 3 years of experience in hematology and oncology.
- Certification in Oncology Nursing preferred or commitment to obtain certification when eligible.
- Current certification in Basic Cardiac Life Support.
- Commitment to collaborative practice and a patient-centered approach to care.
