Job Summary:

The Canadian Red Cross is seeking a highly skilled and compassionate Support Transition Assistant to join our team. As a Support Transition Assistant, you will play a vital role in supporting individuals and their families as they transition from hospital to home.

Key Responsibilities:

  • Organize transportation for individuals from hospital to home, utilizing local transportation services or driving a provided vehicle as needed.
  • Assist individuals in understanding and adhering to hospital discharge instructions, and develop personalized care plans.
  • Perform small errands or arrange for delivery to promote individuals' recovery at home, in accordance with established service plans.
  • Plan, organize, or perform household tasks (cooking, cleaning, laundry, etc.) in accordance with established service plans and in anticipation of hospital discharge, as needed.
  • Foster participant and caregiver engagement, providing relevant information on transition-related topics.
  • Make recommendations and assist individuals, caregivers, and healthcare providers in addressing and resolving safety concerns.
  • Accompany elderly individuals who experience loneliness, as needed.
  • Conduct risk assessments for falls at individuals' homes using a standardized risk assessment tool.
  • Assist individuals in removing obstacles to their well-being and reducing risks by facilitating access to various resources (pharmacy blister packs, assistive devices, referrals, etc.) and ensuring they attend follow-up appointments.
  • Orally communicate concerns, observations, or needs of individuals to the program coordination responsible person or their designated substitute, and enter this information into the appropriate database, in accordance with documentation standards.

Requirements:

  • Completion of at least two years of post-secondary education with a specialization in social work, gerontology, therapeutic aid, or a related field, and possession of at least three years of relevant professional experience or an equivalent combination of education and experience.
  • Knowledge of medical and technical terminology is an asset. If the selected candidate does not possess this type of knowledge, they must acquire practical knowledge of medical and technical terminology as part of their duties.
  • Proficiency in English and French.
  • Proficiency in Microsoft Office.
  • Knowledge of a customer information management system (CIMS) or other databases is an asset.
  • Valid driver's license and access to a reliable vehicle.

Work Environment:

  • The selected candidate will work Monday to Friday in an office environment, hospitals, within the community, in organization vehicles, and at individuals' homes.
  • This work may require bending, stretching, repetitive movements, or climbing, pushing a person in a wheelchair, and assisting them to enter or exit a vehicle.
