Position Overview
We are seeking a compassionate and dedicated Transition Support Specialist to join our team. In this role, you will play a vital part in assisting individuals transitioning from hospital care back to their homes. Your responsibilities will encompass providing practical support, coordinating transportation, and facilitating access to essential resources.
Key Responsibilities
- Coordinate transportation arrangements for individuals returning home from the hospital, utilizing local transport services or driving an organization-provided vehicle as needed.
- Provide guidance and support to individuals in understanding and adhering to discharge instructions from healthcare providers. Develop personalized care plans tailored to their specific needs.
- Assist with essential errands and deliveries to ensure individuals have access to necessary items such as food, medication, medical supplies, and equipment.
- Perform household tasks such as cooking, cleaning, and laundry to support individuals' recovery and well-being.
- Engage with participants and their caregivers by providing relevant information about the transition process from hospital to home.
- Identify and address potential safety concerns within individuals' homes, collaborating with healthcare providers and other stakeholders to ensure a safe and supportive environment.
- Offer companionship and support to isolated seniors as needed.
- Conduct fall risk assessments at individuals' homes using standardized assessment tools.
- Facilitate access to various resources such as pharmacy blister packs, assistive devices, and referrals to healthcare professionals. Ensure individuals attend follow-up appointments.
- Communicate observations, concerns, and individual needs to the program coordinator or designated substitute, accurately documenting information in accordance with established procedures.
Qualifications
- Completion of at least two years of post-secondary education in a field such as social service, gerontology, therapeutic aid, or a related discipline. A minimum of three years of relevant work experience is preferred, or an equivalent combination of education and experience.
- Familiarity with medical and technical terminology is beneficial. If selected candidates lack this knowledge, they will be provided with the necessary training to acquire practical understanding.
- Fluency in both English and French.
- Proficiency in computer tools and a working knowledge of the Microsoft Office suite.
- Experience with customer information management systems (CIMS) or other databases is an asset.
- Possession of a valid driver's license and access to a reliable vehicle.
