The Client Care Coordinator plays a pivotal role within BC Mental Health & Substance Use Services, focusing on the comprehensive management of client care. This position is integral to ensuring that clients receive high-quality, individualized care tailored to their unique needs.
Key Responsibilities
- Lead and coordinate activities related to client care, implementing innovative strategies for service delivery. Collaborate with interdisciplinary teams to set appropriate care goals and develop personalized care plans that address the client's holistic needs.
- Support staff in adhering to care standards and organizational policies. Work collaboratively with clients and team members to ensure consistency in care delivery, evaluating services to meet program requirements and addressing any operational challenges.
- Participate in initial client evaluations and admission discussions, providing insights on care practices and administrative decisions. Conduct thorough assessments of client needs and prioritize care requirements.
- Monitor client progress and provide consultation to healthcare team members. Foster interdisciplinary collaboration to address treatment concerns and ensure continuity of care, while also assisting with therapeutic interventions.
- Build and maintain respectful therapeutic relationships with clients. Engage clients in their treatment planning, empowering them to take an active role in managing their health and well-being.
Qualifications
- Graduation from an accredited School of Nursing with current registration as a Registered Nurse or Registered Psychiatric Nurse.
- Completion of relevant specialty nursing training and a minimum of three years of recent experience in a related clinical setting, along with one year of administrative or supervisory experience.
- Valid driver's license and access to a vehicle may be necessary for local travel.
- Successful completion of Occupational First Aid II (or equivalent) is required.
Skills and Expertise
- In-depth knowledge of primary care practices, mental health disorders, and substance use treatment.
- Proficiency in evidence-based nursing practices, including pharmacology and therapeutic techniques.
- Strong leadership and communication skills, with the ability to collaborate effectively with diverse stakeholders.
- Commitment to understanding and addressing the unique needs of Indigenous populations and the principles of reconciliation.
