Job Title: Mental Health and Substance Use Specialist

We are seeking a skilled Mental Health and Substance Use Specialist to join our Treatment Support and Recovery (TSR) team at the Penticton Health Centre.

About the Role

The successful candidate will have experience in substance use support and will work closely with the Substance Use Connections Team. As a member of our ICM team, you will provide health and social services in the community setting and offer outreach support to vulnerable populations.

Key Responsibilities
  • Conduct ongoing assessments of clients' conditions and determine their needs, including identifying early symptoms of relapse.
  • Provide comprehensive bio/psycho/social services, including a continuum of options designed to optimize clients' recovery and ability to function.
  • Offer highly involved case management, counseling, and other forms of direct service, and assist clients in accessing services to deal with treatment and recovery issues.
  • Arrange access to specialized services, such as substance use, forensic/criminal justice, developmental disability, and acquired brain injury.
  • Document assessments, treatment formulations, care plans, and progress notes according to professional standards and established guidelines.
  • Establish effective working linkages with service providers, including community agencies, hospitals, residential care facilities, primary care physicians, and psychiatrists.
