We are seeking a skilled Mental Health and Substance Use Specialist to join our team at the Penticton Health Centre. As a key member of our Treatment Support and Recovery (TSR)-Intensive Case Management (ICM) team, you will play a vital role in providing health and social services to individuals in our community.
About the RoleThe successful candidate will have experience working with individuals who struggle with substance use and mental health issues. You will work closely with our ICM team to provide assessment, treatment, case management, and community support to clients. Your primary focus will be on helping individuals achieve their recovery goals and improve their overall well-being.
Responsibilities- Conduct ongoing assessments of clients' conditions and determine their needs, including identifying early symptoms of relapse.
- Provide comprehensive bio/psycho/social services to optimize clients' recovery and ability to function.
- Develop and implement individualized treatment plans, including crisis management, problem-solving, and coping skills.
- Collaborate with community agencies, hospitals, and other service providers to ensure seamless care coordination.
- Document assessments, treatment formulations, care plans, and progress notes according to professional standards.
To be successful in this role, you will need:
- A Bachelor's degree in an Allied Health, Behavioural, or Social Science field relevant to the position.
- Two years' recent experience in a mental health and substance use environment or an equivalent combination of education, training, and experience.
- A valid B.C. driver's license.
