The Transitional Care Services Associate role at the Canadian Red Cross presents a unique opportunity to become part of a compassionate team dedicated to assisting clients as they move from hospital settings back to their homes. In this essential position, you will play a crucial role in ensuring the safety and comfort of clients and their families during this pivotal transition.

Key Responsibilities:

  • Support clients throughout the transition phase by adhering to established care plans.
  • Perform errands and manage household tasks, including meal preparation and ensuring a secure living environment.
  • Conduct safety evaluations and address any identified risks.
  • Encourage engagement between clients and caregivers, as well as facilitate referrals to local community resources.
  • Provide companionship within professional boundaries and communicate any concerns to the Program Coordinator.

Qualifications:

  • Certification in Standard First Aid and CPR-C.
  • Proficiency in English; knowledge of French is a valuable asset.
  • Experience working with elderly individuals is preferred.
  • Excellent interpersonal and problem-solving abilities.
  • Capability to work autonomously as well as collaboratively within a team.

Work Environment:

  • Primary responsibilities are performed in the homes of clients.
  • Standard working hours are Monday to Friday from 8:30 AM to 4:30 PM.
  • Flexibility for occasional irregular hours may be necessary within operational times.
