Home Health Educator

Vancouver Coastal Health (VCH) is seeking a passionate and dedicated Home Health Educator to join our team. In this role, you will play a vital part in empowering individuals and families within the Home Health program by providing comprehensive education and support.
As a Home Health Educator, you will leverage your clinical expertise and educational skills to deliver engaging and impactful learning experiences. You will collaborate with interdisciplinary teams, patients, and caregivers to promote health literacy, self-management, and positive health outcomes.
Responsibilities:- Develop and implement evidence-based educational programs tailored to the specific needs of Home Health clients.
- Conduct individual and group education sessions on a wide range of topics related to home health care, chronic disease management, and healthy living.
- Assess client learning needs and provide individualized instruction and support.
- Utilize various teaching methodologies and resources to enhance the learning experience.
- Collaborate with nurses, social workers, and other healthcare professionals to ensure seamless care coordination.
- Evaluate the effectiveness of educational programs and make necessary adjustments.
- Stay current on best practices in home health education and related fields.
- Demonstrated success in implementing educational initiatives within a healthcare setting.
- Comprehensive technical knowledge of client information systems and tools associated with the Home Health program, including Inter Rai-HC and PARIS.
- Clinical experience and knowledge of the Home Health Sector.
- Expertise in educational theory, research, technology, and best practices within healthcare.
- Exceptional group facilitation, team building, negotiation, conflict resolution skills, and an interactive teaching style.
- Ability to foster creativity and innovation in learning and development strategies.
- Superior written communication abilities and proficiency in computer software programs such as Microsoft Office Suite and Outlook.
- Advanced process design and project management skills applied to learning and development initiatives.
