We are seeking a skilled Licensed Practical Nurse Liaison to join our Home Health team at Fraser Health Authority. As a key member of our team, you will play a vital role in providing high-quality patient care and facilitating smooth transitions between acute care and community-based services.
Key Responsibilities- Assess and monitor patients' needs, identifying areas for support and developing personalized care plans.
- Collaborate with interdisciplinary teams to ensure seamless transitions and effective discharge planning.
- Provide education and support to patients and their families, promoting independence and self-management.
- Conduct thorough assessments and screenings to determine appropriate care and services, including home support, nursing care, and residential care.
- Develop and maintain accurate and comprehensive patient records, ensuring timely and effective communication with healthcare teams.
- Participate in quality improvement initiatives, contributing to the development and implementation of best practices and policies.
- Graduation from an approved Practical Nursing program, with a minimum of one year of recent related acute care, residential, and/or community-based clinical experience.
- Current practicing registration as a Licensed Practical Nurse with the British Columbia College of Nurses and Midwives (BCCNM).
- Valid BC Driver's license and access to a personal vehicle for business-related purposes.
