About the Role

Vancouver Coastal Health is seeking an Occupational Therapist to join our Home Health team. As an Occupational Therapist, you will work with clients and families to address functional impairments and acute/chronic diseases in community, ambulatory, and institutional settings.

Key Responsibilities
  • Plan, deliver, evaluate, and provide care coordination to address client needs
  • Collaborate with family members, inter-disciplinary team members, and other healthcare professionals to address client needs
  • Contribute to the organization and delivery of occupational services through quality assurance initiatives, meetings, research, and education
About Home Health Programs

Home Health programs provide a range of healthcare and support services to individuals living in the community with acute, chronic, palliative, or rehabilitative healthcare needs.

Client Population

Home Health clients typically have ongoing health issues and/or complex personal and/or social issues, requiring care planning and coordination of appropriate resources. Clients include frail elderly, younger clients with disabilities, and adults with chronic illnesses requiring supports to maintain/promote independence.

Qualifications
  • Current full registration with the College of Occupational Therapists of British Columbia (COTBC)
  • Eligible for membership in the Canadian Association of Occupational Therapists (CAOT)
  • Valid BC Driver's license and access to a personal vehicle for business-related purposes
Knowledge, Skills, and Abilities
  • Basic knowledge of evidence-based clinical practice related to designated program/area
  • Basic knowledge of occupational therapy standardized tests and measures
  • Broad knowledge of occupational therapy theory and practice within a client and family-centered model of care
  • Basic knowledge of research process and methodology
  • Broad knowledge of other healthcare disciplines and their role in client care
