Job Summary

We are seeking a skilled and compassionate Registered Practical Nurse to join our team at Halton Healthcare. As a key member of our healthcare team, you will be responsible for providing high-quality patient care and contributing to a positive and supportive work environment.

Key Responsibilities
  • Provide direct patient care, including administering medications, performing physical assessments, and developing and implementing individualized care plans.
  • Collaborate with interdisciplinary teams to coordinate patient care and ensure seamless transitions between departments.
  • Participate in quality improvement initiatives and contribute to the development of policies and procedures to enhance patient care and safety.
  • Communicate effectively with patients, families, and healthcare teams to ensure informed decision-making and optimal patient outcomes.
  • Stay up-to-date with the latest medical research, technologies, and best practices to provide evidence-based care.
Requirements
  • Current registration with the College of Nurses of Ontario.
  • Current Heart & Stroke Approved BLS Provider Card upon hire.
  • Experience working with and advocating for the elderly.
  • Medication administration certification and experience.
  • Physical assessment and wound care skills.
  • Enteral feeding experience and maintenance of IV lines.
  • Experience with electronic documentation.
  • Satisfactory employment and attendance record.
