About the Role:
Vancouver Coastal Health is seeking a skilled Nurse Case Manager to join our Home Health team at Pacific Spirit Community Health Centre. As a key member of our interdisciplinary team, you will play a vital role in providing high-quality care to our clients.
Key Responsibilities:
- Manage a caseload of clients with complex health needs, providing comprehensive assessments, care planning, and coordination of services.
- Collaborate with healthcare professionals, community agencies, and clients to ensure seamless care transitions and optimal outcomes.
- Develop and maintain strong relationships with clients, families, and healthcare providers to ensure effective communication and care coordination.
- Provide education and support to clients and families on chronic disease management, self-care, and health promotion.
- Participate in quality improvement initiatives and contribute to the development of policies and procedures that enhance client care.
Requirements:
- Current practicing registration as a Registered Nurse or Registered Psychiatric Nurse with the British Columbia College of Nurses and Midwives.
- Three years' recent clinical nursing experience, preferably in acute care or home health.
- Excellent communication, interpersonal, and problem-solving skills.
- Ability to work independently and as part of a team.
- Computer literacy and proficiency in electronic medical records.
