Youth Transition Coordinator

We are seeking a highly motivated and experienced Youth Support Specialist to join our team at YMCA BC. The successful candidate will provide vulnerable youth with the necessary community supports and life skill development to achieve educational attainment and future success.
Key Responsibilities- Develop and implement youth-centered plans to ensure a safe and welcoming environment for participants.
- Collect and analyze information to confirm eligibility and complete participant intake and/or assessment to inform ongoing participant supports.
- Design, implement, and evaluate individual action plans, in-person services, and workshops as per guidelines and funder mandates.
- Perform case management as per guidelines and funder mandates.
- Consult with social workers, school administration, and other professionals as needed.
- Coordinate peer mentorship opportunities and relationships.
- Maintain knowledge of relevant services and community resources.
- Engage with schools, community agencies, and other organizations to maximize services and resources for participants.
- Adhere to all Risk Management and Occupational Health and Safety requirements.
- Input data accurately and timely into program designated database systems.
- Work within defined program budgets.
- Maintain accurate participant files in accordance with funder and YMCA guidelines.
- Collect demographic, output, and outcome data for evaluation purposes.
- Post-secondary degree/diploma in Child and Youth Care Counselling, Education, Social Work, Recreation Management, or a combination of related education and experience.
- 2 years of experience working directly with vulnerable youth.
- Experience in client intake and assessment, case management, and workshop facilitation.
- Experience within a team environment.
- Non-Violent Crisis Intervention, Motivational Interviewing, Suicide Intervention Certification an asset.
- Valid driver's license and access to a vehicle required.
- Working knowledge of issues/practices related to the social determinants of health, youth development, individual assessment, case management, group work, group facilitation, program planning, and outcome evaluation.
- Working knowledge in culturally sensitive and trauma-informed practice, and strength-based approach.
- Excellent organizational and time management skills.
- Excellent communication, interpersonal, and team skills.
- Working knowledge of computer skills (Windows, Microsoft Teams, Office, and Outlook).
- Experience working with youth 12-19.
- Well-developed interpersonal and relationship-building skills.
- Experience and sensitivity in dealing with members of different cultural and racial backgrounds.
- Successful candidates will be required to provide a current and satisfactory Criminal Reference Check/Vulnerable Sector Search.
- Current Emergency First Aid and CPR-C with AED certificate.
