Registered Nurse, Transition Line

We are seeking a highly skilled and dedicated Registered Nurse to join our team in a Transition Line role. As a key member of our healthcare team, you will be responsible for providing high-quality patient care and contributing to the development of our nursing staff.
Key Responsibilities- Assess patients and develop individualized care plans to meet their unique needs.
- Collaborate with interdisciplinary teams to ensure seamless care coordination and communication.
- Provide education and support to patients and their families to promote health and wellness.
- Participate in quality improvement initiatives to enhance patient outcomes and nursing practices.
- Current registration as a Registered Nurse with the British Columbia College of Nurses and Midwives.
- Completion of a post-graduate specialty nursing course or program.
- Excellent communication and interpersonal skills.
- Ability to work effectively in a fast-paced environment.
