Job Summary:
The Activity Instructor II (AI2) is a key member of the Prairie Mountain Health team, responsible for providing mobile community-based services for individuals with mental health problems. This role involves assessing, developing, and implementing individual and group programming to engage clients with their environment.
Key Responsibilities:
- Engage with clients to develop a therapeutic working relationship and assess individual strengths, interests, goals, barriers to goal achievement, functional, and resource needs.
- Develop a strength-based plan with the client that promotes empowerment, independence, and self-responsibility, working in collaboration with the client and their support network.
- Implement the activity plan, evaluate the client's performance in selected tasks, and provide constructive feedback that promotes personal growth.
- Provide ongoing teaching, training, guidance, and direct feedback in the implementation of a client plan, monitoring and modifying the plan as needed.
- Develop and post an activity schedule to enhance client participation in planned activities, evaluating and re-prioritizing the schedule as necessary.
- Participate in multi-disciplinary planning, communicate with team members, and assume responsibility for client assignments.
- Consult with Occupational Therapists, Community Mental Health Workers, and other team members regarding activities for clients with specific functional impairments.
Group Facilitation and Community Engagement:
- Work with individuals and small groups to identify learning and working needs, developing individual and small group awareness of resources through information sharing and support.
- Link individuals and small groups to community agencies and resources to assist in achieving personal goals, identifying and providing opportunities for expanded life experiences to increase self-confidence and self-esteem.
- Organize and implement group activities, providing instruction for selected tasks to develop knowledge and abilities.
Health Promotion and Disease Prevention:
- Participate in regular planning to organize and deliver programs that promote health, communicating with clients, internal, and external partners to increase awareness, knowledge, and understanding of mental health issues.
- Plan and deliver health-promoting activities with clients, families, and internal and external partners, participating in mental health promotion through education and public relations activities.
Crisis Management:
- Facilitate ongoing client education and training related to safe work practices, mediating risk and addressing safety concerns for clients within the environment.
- Assess the potential for crisis situations and intervene to de-escalate/defuse situations, engaging the client and relevant others in the development and documentation of a work-related crisis plan.
Program-Related Responsibilities:
- Maintain activity program statistics and provide monthly reports, recording assessments, activity plans, and evaluations as per program requirements.
- Ensure a sufficient inventory of supplies are available to meet activity program requirements, maintaining records of supplies used and requisitioning replacements as needed.
- Ensure equipment is used safely, stored appropriately, and maintained in good working order, advising the Program Manager of recommendations for capital budget expenditures.
- Maintain confidentiality in all matters pertaining to patients and their families, prioritizing workload by utilizing sound organizational skills.
