Community Outreach Specialist

We are seeking a highly skilled and compassionate Community Outreach Worker to join our team at Nova Scotia Health and IWK Health. As a key member of our Mental Health and Addictions team, you will play a vital role in providing clinical services to individuals and families in need.
Key Responsibilities:
- Provide a range of clinical services, including screening, assessment, brief intervention, case management/navigation, individual and group treatment.
- Work in both clinic and community settings, navigating and advocating for clients and their families.
- Develop and maintain collaborative relationships with community organizations and agencies to ensure seamless service delivery.
- Utilize principles of harm reduction, community development, and psychosocial rehabilitation to support clients in achieving their goals.
- Serve as a resource to colleagues, clients, and family members, providing education and support to promote recovery and well-being.
Requirements:
- Bachelor of Social Work degree.
- Registration or eligibility for registration with a professional regulatory body.
- One year post-BSW clinical experience in Mental Health and Addictions.
- Screening and risk assessment skills, including suicide risk assessment.
- Ability to work independently, assess situations, and implement evidence-based plans of care.
- Working knowledge of the DSM.
- Experience in offering brief, supportive intervention strategies focusing on harm reduction and recovery.
- Ability to navigate and de-escalate situations, offer crisis support, and facilitate groups.
- Strong communication and interpersonal skills, with the ability to build and maintain collaborative relationships.
- Competencies in other languages an asset, French preferred.
