Resident Services Coordinator

We are seeking a skilled and compassionate Resident Services Coordinator to join our team at Shannex Incorporated.
As a key member of our care team, you will be responsible for providing exceptional support to our residents and their families. Your primary focus will be on ensuring the highest level of care and service delivery, while promoting a safe and respectful environment for all.
Key Responsibilities:
- Prepare reports, records, and statistics as needed to support care planning and decision-making.
- Participate in interdisciplinary team meetings to discuss resident care and service needs.
- Collaborate with residents, families, and healthcare professionals to develop and implement personalized care plans.
- Conduct tours of our facilities and engage with prospective residents and their families to provide information and support.
- Coordinate admission, transfer, and discharge processes to ensure seamless care transitions.
- Prepare funding requests and reports to support resident care and service needs.
About You:
We are looking for a qualified and experienced Resident Services Coordinator who possesses:
- A current license as a Licensed Practical Nurse (LPN) or Registered Nurse (RN) with the Nova Scotia College of Nursing.
- CPR certification.
- Previous experience working in long-term care or with seniors.
- Strong computer literacy skills and ability to communicate effectively.
- Knowledge of available community programs and supports.
