Pacific Coast Community Resources seeks a skilled Nurse Care Specialist to provide high-quality care and support to individuals with diverse abilities. As a key member of our team, you will work collaboratively with healthcare professionals, families, and community resources to promote independence, self-expression, and community integration.

Key Responsibilities:

  • Develop and implement individualized program plans to address the total needs of each supported person.
  • Collaborate with healthcare professionals to provide comprehensive care and support.
  • Communicate effectively with families, caregivers, and community resources to ensure seamless care coordination.
  • Monitor and evaluate care plans to ensure optimal outcomes.
  • Provide education and training to supported persons and their families on health and wellness topics.

Requirements:

  • Current registration with the BCCNM or EMALB as an RN or LPN.
  • Previous experience in a healthcare setting or recent graduation from a nursing program with a multidisciplinary approach.
  • Experience working with youth with significant substance use/addiction & mental health challenges.
  • Experience in Individualized Program Plans (IPP), Person-Specific Planning (PSP), and Health Care Planning (HCP).
  • Current CPR "C" certification.
  • Valid Class 5 driver's license; Class 4 driver's license preferred.
