We are seeking a highly skilled and compassionate Mental Health Support Worker to join our team at the Provincial Assessment Centre. As a key member of our interdisciplinary team, you will play a vital role in providing support and care to individuals with complex mental health and substance use challenges.
Key Responsibilities- Monitor clients and observe their behaviors and symptoms, documenting observations in accordance with standard procedures.
- Participate in the admission, case planning, and discharge process for clients, including conducting interviews and gathering relevant information.
- Establish rapport with clients and provide emotional support and counseling, assisting with problem-solving and daily living activities.
- Coordinate services with medical professionals and community resources, acting as an advocate for clients and providing information on available services.
- Plan and lead groups, such as sports and exercise, social support groups, and social activities, to support skill development and client engagement.
- Maintain client charts and databases, ensuring accurate and up-to-date information.
- Diploma from a recognized Social Services Support Worker educational program or equivalent combination of education and experience.
- Valid driver's license and ability to travel locally.
- Knowledge of mental health issues, treatment, and recovery principles.
- Ability to establish and maintain rapport with clients, observe and recognize changes in clients, and deal effectively with conflict situations.
- Commitment to upholding the shared responsibility of creating lasting and meaningful reconciliation in Canada.
