St. Joseph's Home Care is seeking a skilled Registered Nurse to join our team as a Wound Care Nurse. As a key member of our interdisciplinary health team, you will provide high-quality wound and ostomy care to clients and caregivers in their homes and alternative settings.
Key Responsibilities:- Utilize the nursing process to provide effective wound and ostomy care, adhering to best practices and protocols.
- Collaborate with the health team to develop and implement cost-effective, high-quality wound care plans.
- Stay current with wound care research and protocols, applying this knowledge to practice.
- Create and implement individualized plans of care for clients requiring wound care, in consultation with physicians, CCAC case managers, and other healthcare professionals.
- Promote client and caregiver independence, considering their needs, strengths, and weaknesses.
- Utilize available resources to facilitate problem-solving, decision-making, and access to appropriate resources.
- Participate in the evaluation and refinement of wound care protocols and programs.
- Bachelor of Science Degree from an accredited university.
- Current and unrestricted Registered Nurse license.
- Member in good standing with the Ontario College of Nurses.
- A current Ontario driver's license in good standing.
- Proven ability to utilize MS Office software, including Excel spreadsheets.
- Knowledge of community supports, home care, acute, and long-term care settings.
- Familiarity with Procura software (preferred).
- Certification as a wound care nurse or equivalent courses (recommended).
- Minimum of one year recent experience in home care (preferred).
