Job Title: Wound Care Nurse

St. Joseph's Home Care is seeking a skilled Wound Care Nurse to join our team. As a Registered Practical Nurse, you will provide high-quality care to clients with predictable outcomes, within the framework of our policies and procedures and the standards of practice prescribed by the College of Nurses of Ontario.

Responsibilities:
  • Provide quality nursing care to clients and families, within your scope of practice, for clients with predictable outcomes.
  • Utilize the nursing process effectively to provide high-quality nursing care for clients and/or caregivers in all client situations.
  • Follow plan of nursing care, including discharge plan, and evaluate and modify plan of care on an ongoing basis.
  • Provide nursing care primarily within the Hamilton Visiting Program, but also provide nursing care to clients within the Integrated Client Care Program if needed.
  • Maintain current competence and participate in continuing professional development opportunities to meet client care needs.
  • Act as an advocate for clients/caregivers by interpreting information, supporting decisions, and facilitating communications.
Qualifications:
  • Current RPN registration with the College of Nurses of Ontario with no restrictions on licence or hearings pending.
  • Practical Nursing Diploma from an accredited college program with evidence of IV medication administration as part of the basic educational curriculum.
  • Wound care certificate from an accredited institution.
  • Basic Cardiac Life Support (BCLS).
  • Ability to function to the full scope of practice as per the College of Nurses of Ontario.
  • Experience conducting comprehensive nursing assessments.
  • PPS, ESAS.
  • Ability to effectively use software for client documents, word processing, and communication.
  • Committed to customer service.
  • Demonstrated commitment to professional development and continuing education.
  • Good work performance and attendance record are essential.
  • Demonstrated commitment to the values of Dignity, Respect, Service, Justice, Responsibility, and Enquiry.
  • Original and current (within six months) Police Check with Vulnerable Sector Screening.
  • Valid Driver's License. Proof of $1M automobile liability insurance.
  • COVID 19 Vaccination.
