Job Title: RPN - Wound Care

St. Joseph's Home Care is committed to delivering high-quality, compassionate care to individuals in their homes and/or alternative settings. As a Registered Practical Nurse (RPN) in our Wound Care team, you will play a vital role in enhancing the lives of our clients by providing a range of care services to those with predictable outcomes.

Responsibilities:
  • Function independently in the community, providing quality nursing care to clients and families with predictable outcomes.
  • Utilize the nursing process effectively to provide high-quality nursing care for clients and/or caregivers in all client situations.
  • Follow the plan of nursing care, including discharge plans, and evaluate and modify the plan of care on an ongoing basis through consultation and liaison with clients, the LHIN, physicians, and other multidisciplinary team members.
  • Provide nursing care primarily within the Hamilton Visiting Program, but also provide nursing care to clients within the Integrated Client Care Program if needed.
  • Maintain current competence and participate in continuing professional development opportunities to obtain advanced or specialized nursing skills to meet client care needs.
  • Act as an advocate for clients/caregivers by interpreting information, supporting decisions, and facilitating communications.
Qualifications:
  • Current RPN registration with the College of Nurses of Ontario with no restrictions on license or hearings pending.
  • Practical Nursing Diploma from an accredited college program with evidence of IV medication administration as part of the basic educational curriculum.
  • Practical Nursing Diploma from an accredited college program and IV medication administration certificate from an accredited college program if this content was not included in the basic diploma educational curriculum.
  • Practical Nursing Certificate from an accredited college program, a medication administration certificate, and an IV medication administration certificate from an accredited college program.
  • Must have wound care certificate from an accredited institution.
  • Current registration with the College of Nurses of Ontario.
  • Basic Cardiac Life Support (BCLS).
  • Ability to function to the full scope of practice as per the College of Nurses of Ontario.
  • Experience conducting comprehensive nursing assessments.
  • PPS, ESAS.
  • Ability to effectively use software for client documents, word processing, and communication.
  • Committed to customer service.
  • Demonstrated commitment to professional development and continuing education.
  • Good work performance and attendance record are essential.
  • Demonstrated commitment to the values of Dignity, Respect, Service, Justice, Responsibility, and Enquiry.
  • Demonstrates the following competencies: Service and Mission Focus, Achievement, Teamwork, Learning and Innovation, Communication, Conflict Resolution, and Resilience.
  • Original and current (within six months) Police Check with Vulnerable Sector Screening.
  • Valid Driver's License. Proof of $1M automobile liability insurance. Able to drive locally and on the highway.
  • Must be COVID-19 vaccinated.
