Dietitian - Primary Care Wellness Support Specialist

We are seeking a dedicated and experienced Dietitian to join our Primary Care Support Team at Group Health Centre. As a key member of our interdisciplinary team, you will conduct comprehensive nutritional assessments, develop personalized care plans, and provide education to patients and their families.
Key Responsibilities:- Conduct nutritional assessments and develop care plans for patients of all ages
- Provide education and support to patients and their families on healthy eating and nutrition
- Collaborate with healthcare professionals to deliver high-quality care and promote patient empowerment
- Develop and implement nutritional care plans tailored to various medical conditions and age groups
- Graduate of a recognized university with a Bachelor of Science in Nutrition or equivalent
- Registered as a member of the College of Dietitians of Ontario in good standing
- Excellent interpersonal and communication skills
- Ability to work effectively as a member of an interdisciplinary team
- Certified Diabetes Educator (CDE) Certificate
- Experience working with patients of all ages and medical conditions
- Competitive salary range: $64,000 - $80,000 annually
