Job Summary:
We are seeking a dedicated and experienced Dietitian to join our Primary Care Support Team at Group Health Centre. As a key member of our interdisciplinary team, you will conduct comprehensive nutritional assessments, develop personalized care plans, and provide education to patients and their families.
Key Responsibilities:
- Conduct comprehensive nutritional assessments and develop personalized care plans for patients and their families.
- Provide education and support to patients and their families on nutrition and wellness.
- Collaborate with healthcare professionals to deliver high-quality care and ensure the highest standard of care.
- Develop and implement appropriate nutritional care plans tailored to various age groups and medical conditions.
Requirements:
- Graduate of a recognized university with a Bachelor of Science in Nutrition or equivalent and successful completion of an accredited Dietetic Internship Program.
- Registered as a member of the College of Dietitians of Ontario in good standing.
Preferred Qualifications:
- Certified Diabetes Educator (CDE) Certificate and asset.
What We Offer:
- $64,000 - $80,000 annually (commensurate with experience).
