Job Summary:

The Group Health Centre is seeking a skilled Dietitian to join our team. As a Dietitian, you will be responsible for providing comprehensive nutrition assessment, care planning, and education to patients and families in both group and individual settings.

Key Responsibilities:

  • Collaborate with the interdisciplinary healthcare team to develop and implement patient care plans
  • Conduct comprehensive nutrition assessments and provide personalized nutrition education to patients and families
  • Develop and maintain relationships with healthcare professionals and other agencies to ensure seamless patient care
  • Participate in quality improvement initiatives and contribute to the development of standards and policies

Requirements:

  • Registered Dietitian with a degree in Human Applied Nutrition or equivalent
  • Active membership in the Ontario College of Dietitians in good standing
  • Excellent interpersonal and communication skills
  • Ability to work effectively in a team environment

Preferred Qualifications:

  • Certified Diabetes Educator (CDE) Certificate
  • Experience working with patients with diabetes and other chronic conditions
