Job Summary

The Registered Practical Nurse at St. Joseph's Care Group plays a vital role in providing high-quality care to clients in the Adult Day Program. As a key member of the healthcare team, you will be responsible for monitoring clients' health status, administering minor medical care, and providing programming that includes group and client-specific activities.

Key Responsibilities

  • Monitor clients' health status and administer minor medical care as needed
  • Develop and implement individualized care plans in conjunction with the healthcare team
  • Provide programming that includes group and client-specific activities to promote physical and emotional well-being
  • Communicate effectively with family/caregivers to ensure continuity of care
  • Collaborate with the healthcare team to ensure seamless delivery of care

Requirements

  • Current registration with the College of Nurses of Ontario (CNO)
  • Pharmacology certificate
  • Minimum two years experience working with older adults living with dementia and responsive behaviors
  • Education in Gerontology, Alzheimer's Disease, or other related dementias an asset
  • Excellent interpersonal, oral, and written communication skills
  • Ability to exercise good judgment and make sound decisions
  • Must be able to meet the physical demands of the position
  • Demonstrated regular attendance at work

Conditions of Employment

  • This position is conditional upon a satisfactory Police Vulnerable Sector Check (PVSC) where indicated, to ensure the absence of relevant criminal convictions
  • Offers of employment made to external candidates are conditional upon providing proof of COVID-19 vaccination as per the organization's policy or proof of valid medical exemption, prior to the offered effective date
