Job Summary:

In alignment with the standards of practice of the British Columbia College of Nurses and Midwives (BCCNM) and the Mission and Values of Fraser Health, this role involves working collaboratively within an inter-professional team to deliver direct patient care at the Urgent Primary Care Centre, ensuring timely and longitudinal primary care to vulnerable clients.

Key Responsibilities:

  • Provide direct nursing care to clients, including assessment and collection of information from various sources.
  • Establish therapeutic relationships with clients to ensure autonomy in decision-making.
  • Collaborate with healthcare providers to resolve client care issues and coordinate services.
  • Develop comprehensive client care plans in collaboration with the inter-professional team.
  • Conduct outreach nursing care to clients in their home setting when needed.
  • Provide health counseling, education, and wellness promotion to clients.
  • Maintain clinical records and stay updated on current standards of practice.

Requirements:

  • Current registration as a Registered Nurse with BCCNM.
  • Minimum one year of recent clinical experience in acute care or related areas.
  • Valid BC Driver's license and access to a personal vehicle when required.
  • Strong communication skills and ability to problem-solve effectively.
  • Knowledge of health care services and community resources.
  • Ability to work independently and in a dynamic environment.
