Job Title: Personal Support Worker - Nursing Care

We are seeking a compassionate and dedicated Personal Support Worker to join our team at Transitions Care. As a Personal Support Worker, you will play a vital role in providing high-quality nursing care to our clients in their homes.

Key Responsibilities:
  • Assist with daily living activities, such as bathing, dressing, and grooming
  • Provide personal care, including toileting, feeding, and transferring
  • Assist with medication management and administration
  • Monitor and report any changes in client condition
  • Maintain a safe and clean environment for clients
  • Communicate effectively with clients, families, and healthcare professionals
Requirements:
  • High school diploma or equivalent required
  • First Aid and CPR certification
  • Experience working with seniors or individuals with disabilities
  • Ability to work independently and as part of a team
  • Excellent communication and interpersonal skills
