Position Overview

In alignment with the established vision and values of our organization, the Transition Nurse operates in accordance with the professional practice standards and ethical guidelines set forth by the BC College of Nurses and Midwives (BCCNM), within a model that prioritizes patient and family-centered care.

Under the supervision of the Manager (or designate), the Transition Nurse collaborates effectively with multidisciplinary and multi-agency teams, taking a proactive role in the planning, execution, assessment, and coordination of integrated care plans for clients with complex discharge requirements and service needs.

This role involves comprehensive evaluations of individuals, identifying discrepancies from discharge plans and service requirements, as well as addressing client, caregiver, and systemic challenges.

The Transition Nurse serves as a valuable resource for clients and families, as well as various health service teams, in assessing client needs and determining eligibility for suitable resources.

This position is responsible for communicating any changes in the daily service requirements of clients and coordinating with the multidisciplinary team to ensure that shifts in capacity and utilization are anticipated, monitored, and alternative strategies are devised to optimize the use of health service resources to meet client needs.


Work Schedule: Monday to Friday, Days - 08:30 to 16:30

Required Qualifications:
  • Registration with BC College of Nurses and Midwives as a practicing RN registrant.
  • Two (2) years of recent relevant clinical experience, including a mix of Community and Acute Care nursing experience with clinically complex clients; or an equivalent combination of training and experience.
  • A valid driver's license and access to a reliable personal vehicle are required.
