We are seeking a skilled Licensed Practical Nurse to join our team at AgeCare. As a LPN, you will play a vital role in providing high-quality patient care and contributing to the overall well-being of our residents.

Key Responsibilities
  • Medication Administration: Administer medications to patients as prescribed by physicians, ensuring accurate dosages and monitoring for potential side effects.
  • Patient Assessment and Care Planning: Conduct thorough assessments of patients' physical and emotional needs, developing and implementing individualized care plans to promote optimal health outcomes.
  • Health Education and Promotion: Provide patients and their families with education and support on healthy lifestyle choices, disease management, and wellness strategies.
  • Collaboration with Healthcare Team: Work closely with physicians, nurses, and other healthcare professionals to ensure seamless care coordination and effective communication.
  • Documentation and Record-Keeping: Maintain accurate and up-to-date records of patient interactions, treatments, and outcomes, adhering to established policies and procedures.
Requirements
  • Education: Completion of a diploma or certificate program in practical nursing from a recognized institution.
  • Experience: Previous experience in a long-term care setting or a related field is an asset.
  • Certifications and Licenses: Current licensure as a Licensed Practical Nurse in the province of practice, as well as certification in Basic Cardiac Life Support (BCLS) and CPR.
  • Security and Safety: Completion of a criminal record check, immunization records, and a tuberculosis test.
