Social Worker

Halton Healthcare Services is seeking a skilled Social Worker to join our team in providing high-quality mental health services to our patients. As a Social Worker, you will work collaboratively with our multidisciplinary team to provide comprehensive social work services to inpatients on our Adult Inpatient Mental Health Units.
Key Responsibilities:- Conduct comprehensive social work/psychosocial assessments to identify patient needs and develop individualized treatment plans.
- Provide selected therapeutic interventions, including counseling, education, and group interventions, to patients and families.
- Collaborate with community partners to facilitate connections to community resources and support services.
- Participate in team conferences, interdisciplinary rounds, and educational rounds to ensure seamless care coordination.
- Advocate for patients and families to address systemic barriers and promote high-quality care.
- Master of Social Work degree required.
- Current registration and membership in good standing with the College of Social Workers and Social Service Workers.
- Minimum 3 years' experience in a hospital or community health organization.
- Strong clinical, communication, and collaboration skills.
- Familiarity with Mental Health Act, Consent to Treatment Act, Substitute Decisions Act, and Healthcare Consent Act.
