About the Opportunity

The Continuing Care Coordinator is responsible for assessing, planning, and authorizing services for clients referred to Continuing Care. This role involves working with interprofessional teams to facilitate hospital discharge and support community-dwelling clients. The Coordinator will assess clients' needs, identify goals, and develop plans to meet those goals. They will also authorize services and make referrals to other resources as needed.

The ideal candidate will have a Bachelor's degree in a relevant field, such as Nursing, Social Work, or Occupational Therapy. They will also have a minimum of three years' experience in a community or long-term care setting, with knowledge of community agencies and resources. Excellent assessment, interviewing, and negotiation skills are essential for this role.

Hours and Compensation

This is a permanent full-time position with a 70-hour bi-weekly schedule. Shifts may include days, evenings, weekends, and holidays. The hourly rate is $46.75.
