About the Opportunity

The Care Coordinator is responsible for assessing, planning, and authorizing services for clients referred to Continuing Care. This role involves working with interprofessional teams to facilitate hospital discharge and support community-dwelling clients. The Care Coordinator must be able to identify client goals, unmet needs, and resource requirements, and develop a plan to meet these needs.

The ideal candidate will have a Bachelor's degree in Nursing, Social Work, Occupational Therapy, Physiotherapy, or Nutritional Studies, and current registration with the applicable Nova Scotia Professional Regulatory body. A minimum of three years' experience in a community or long term care setting is required, as well as knowledge of community agencies and resources. Excellent assessment, interviewing, and negotiation skills are essential, as well as the ability to work independently and co-operatively in a busy multidisciplinary situation.

The Care Coordinator will be responsible for authorizing services, making referrals, and linking clients with other professional and volunteer resources. They will work with clients and their care partners to identify goals and develop a plan to meet these needs, and will be responsible for ongoing communication with clients, family, caregivers, care providers, and healthcare teams.

About You
  • Bachelor's degree in Nursing, Social Work, Occupational Therapy, Physiotherapy, or Nutritional Studies
  • Current registration with the applicable Nova Scotia Professional Regulatory body
  • Minimum three years' experience in a community or long term care setting
  • Knowledge of community agencies and resources
  • Excellent assessment, interviewing, and negotiation skills
  • Ability to work independently and co-operatively in a busy multidisciplinary situation
  • Valid driver's license and access to a reliable vehicle
Hours
  • Permanent Full Time; 70 Hours Bi-weekly
  • Shifts may include days, evenings, weekends, and holidays
Compensation and Benefits

$46.75 Hourly
