Continuing Care Coordinator

The Continuing Care Coordinator plays a vital role in assessing, planning, and authorizing services for clients referred to Continuing Care. This position requires strong assessment and interviewing skills, as well as the ability to work independently and collaboratively in a multidisciplinary setting.
The ideal candidate will have a Bachelor's degree in a relevant field, such as Nursing, Social Work, or Occupational Therapy, and a minimum of three years' experience in a community or long-term care setting. They will also possess excellent communication and negotiation skills, as well as the ability to work effectively with clients, families, and healthcare teams.
The Continuing Care Coordinator will be responsible for:
- Assessing clients' needs and developing care plans
- Authorizing services and making referrals to other resources
- Working with interprofessional teams to facilitate hospital discharge and return to the community
- Maintaining ongoing communication with clients, families, and healthcare teams
This is a permanent full-time position, with a 70-hour bi-weekly schedule. The successful candidate will be eligible for our benefits package, which includes health, dental, travel, and life insurance coverage, as well as a defined benefit pension plan.
