The Continuing Care Coordinator is responsible for assessing, planning, and authorizing services for clients referred to Continuing Care. This role involves working with interprofessional teams to facilitate hospital discharge and support community-dwelling clients. The Coordinator will assess clients' needs, identify goals, and develop plans to meet those goals. They will also authorize services and make referrals to other resources as needed.
About YouWe are seeking a highly skilled and experienced professional to fill this role. The ideal candidate will have a Bachelor's degree in Nursing, Social Work, Occupational Therapy, Physiotherapy, or Nutritional Studies, and current registration with the applicable Nova Scotia Professional Regulatory body. A minimum of three years' experience in a community or long term care setting is required, as well as knowledge of community agencies and resources. Excellent assessment, interviewing, and negotiation skills are essential, as well as the ability to work independently and co-operatively in a busy multidisciplinary situation.
Responsibilities- Assess clients' needs and develop plans to meet those needs
- Authorize services and make referrals to other resources as needed
- Work with interprofessional teams to facilitate hospital discharge and support community-dwelling clients
- Develop and maintain relationships with community agencies and resources
- Provide education and support to clients and their families
