Job Summary:

We are looking for a skilled and compassionate Licensed Practical Nurse to provide high-quality care to our clients in their homes. As a Licensed Practical Nurse, you will work collaboratively with our interdisciplinary team to deliver patient-centered care that meets the unique needs of each individual.

Responsibilities:
  • Provide nursing care to clients in their homes, including administering medications, wound care, and vital sign monitoring.
  • Conduct thorough assessments of clients' physical and emotional needs, and develop individualized care plans to address these needs.
  • Collaborate with other healthcare professionals, including physicians, therapists, and social workers, to ensure comprehensive care.
  • Document client progress and care provided in a timely and accurate manner.
  • Participate in ongoing education and training to stay up-to-date on best practices and industry developments.
Requirements:
  • Current licensure as a Licensed Practical Nurse in the province of employment.
  • Minimum 1 year of experience in home care or a related field.
  • Excellent communication and interpersonal skills.
  • Ability to work independently and as part of a team.
  • Valid driver's license and reliable transportation.
