Job Title: Licensed Practical Nurse

Job Summary:

We are looking for a skilled and compassionate Licensed Practical Nurse to provide high-quality patient care in a home care and visiting care agency setting. The successful candidate will have a strong educational background, excellent communication skills, and the ability to work independently in a fast-paced environment.

Responsibilities:
  • Provide patient care in a home care and visiting care agency setting
  • Assess patient needs and develop individualized care plans
  • Administer medications and treatments as prescribed by healthcare professionals
  • Monitor patient progress and report any changes or concerns to healthcare professionals
  • Collaborate with healthcare professionals to ensure comprehensive patient care
  • Maintain accurate and up-to-date patient records
Requirements:
  • Current licensure as a Licensed Practical Nurse in the province
  • Valid driver's license and reliable transportation
  • First Aid and CPR certification
  • Immunization records
  • Child Abuse Registry check
  • Criminal record check
Preferred Qualifications:
  • Experience working in a home care and visiting care agency setting
  • Knowledge of community resources and services
  • Ability to work independently and as part of a team
