Job Title: Licensed Practical Nurse

Job Summary:

We are looking for a skilled and compassionate LPN to provide nursing care to clients in their homes and communities. The successful candidate will work collaboratively with our interdisciplinary team to assess, plan, implement, and evaluate client care.

Responsibilities:
  • Assess client needs and develop individualized care plans
  • Provide nursing care to clients in their homes and communities
  • Collaborate with other healthcare professionals to ensure comprehensive client care
  • Document client progress and care provided
  • Participate in ongoing education and professional development to maintain current knowledge and skills
Requirements:
  • Current licensure as a Licensed Practical Nurse in the province of employment
  • Experience in home and community care an asset
  • Excellent communication and interpersonal skills
  • Ability to work independently and as part of a team
  • Valid driver's license and reliable vehicle
