LPN - Wellness Nurse

At Sunrise Senior Living, we are committed to providing high-quality care to our residents. As a LPN - Wellness Nurse, you will play a vital role in ensuring the overall well-being of our residents.
Responsibilities:- Identify Resident Needs: Conduct regular assessments to identify the ongoing needs and services of our residents, promoting the highest quality resident services.
- Ensure Proper Follow-Through: Participate in the evaluation and documentation of residents with changes in healthcare services and status, ensuring seamless transitions.
- Collaborate with Resident Care Director: Assist in conducting resident evaluations and gathering data at move-in, 30 days post-move-in, semi-annually, and with any change in status.
- Monthly Wellness Visits: Conduct regular wellness visits for all residents, monitoring their health and well-being.
- Incident Reporting: Monitor and follow up with residents who have experienced falls, accidents, or incidents, documenting and reporting as required.
- Communication: Maintain open communication with residents' families and responsible parties to ensure their medical needs are met.
- Medical Support: Contact residents' attending physicians when necessary and upon family request.
- Vital Signs and Weights: Ensure vital signs and weights are obtained monthly for each resident.
- Wellness File Maintenance: Assist in maintaining wellness files according to Sunrise policies and federal and state regulations.
- Medical Supplies: Maintain medical supplies and emergency kits for the community.
- Team Support: Provide clinical support and assistance to community team members as needed.
- Infection Control: Understand and follow infection control practices, partnering with the Resident Care Director and community team to promote infection control standards.
- Medication Management: Regularly monitor each resident's medication and treatment profile to ensure accurate administration and documentation.
- Quality Improvement: Demonstrate knowledge of key quality improvement areas, including Resident Centered Care Model, APIE, Quality Care Indicators and Outcomes, and actively participate in Sunrise Quality Assurance Meetings & Clinical Meetings.
- Regulatory Compliance: Demonstrate knowledge of federal, state, and local resident care and services regulations, Resident Rights, and Resident Assessment/Evaluation process in accordance with state/province regulatory requirements.
Qualifications:
- Education: Graduate of an approved college/school of nursing.
- Licensure: Maintains a current state license as a professional Licensed Practical Nurse/Licensed Vocational Nurse (LPN/LVN).
- Experience: Minimum of one (1) year experience in home health, assisted living, hospital, or long-term care environment.
- Skills: Demonstrates knowledge of nursing practices, techniques, and methods applied to health and wellness resident care, and federal, state, and local regulations.
- Communication: Possesses written and verbal skills for effective communication and the ability to teach and guide team members to provide quality clinical care.
- Organizational Skills: Competent in organizational, time management skills.
- Judgment and Problem-Solving: Demonstrates good judgment, problem-solving, and decision-making skills.
- Computer Skills: Demonstrates proficiency in computer skills, Microsoft Office (Windows, Outlook, Excel), and Sunrise applications with the ability to learn new applications.
- Availability: Ability to work weekends, evenings, and flexible hours, available to our customers at peak service delivery days and times.
