LPN - Wellness Nurse

Sunrise Senior Living is seeking a highly skilled and compassionate Licensed Practical Nurse (LPN) to join our team as a Wellness Nurse. As a key member of our healthcare team, you will be responsible for providing high-quality care and services to our residents, promoting their overall well-being and quality of life.
Key Responsibilities- Resident Care: Identify and address the ongoing needs and services of residents, ensuring the highest quality of care is delivered.
- Healthcare Services: Ensure proper follow-through and participate in evaluations and documentation for residents with changes in healthcare services and status.
- Wellness Visits: Conduct monthly wellness visits for all residents, monitoring their health and well-being.
- Incident Reporting: Monitor and report incidents, such as falls or accidents, and document accordingly.
- Communication: Maintain open communication with residents' families and responsible parties to ensure their medical needs are met.
- Medical Support: Provide clinical support and assistance to community team members as needed.
- Education: Graduate of an approved college or school of nursing.
- Licensure: Current state license as a professional Licensed Practical Nurse/Licensed Vocational Nurse (LPN/LVN).
- Experience: Minimum of one (1) year experience in home health, assisted living, hospital, or long-term care environment.
- Skills: Demonstrated knowledge of nursing practices, techniques, and methods applied to health and wellness resident care, and federal, state, and local regulations.
- Communication: Possesses written and verbal skills for effective communication and the ability to teach and guide team members to provide quality clinical care.
- Competitive Compensation: Selected candidates will be offered competitive compensation based on geographic location, skills, experience, qualifications, and certifications/licenses (where applicable).
- Benefits: Full Extended Health Benefit Package and RRSP package with 3% matching.
- Work Schedule: Fridays and Saturdays, 6 am-2:30 pm, and Sundays and Mondays, 2 pm-10:30 pm.
