POSITION SUMMARY:
St. Joseph's Home Care is committed to delivering exceptional and compassionate care to individuals in their residences and alternative environments, whether on a visiting or shift care basis. Registered Practical Nurses play a vital role in enhancing the quality of life for their clients by offering a spectrum of care with predictable outcomes, adhering to SJHC's policies and procedures and the standards set forth by the College of Nurses of Ontario. Through their dedicated efforts, Registered Practical Nurses foster the healing and autonomy of their clients while promoting the continuous enhancement of community health.RESPONSIBILITIES:
- Operates independently within the community, delivering high-quality nursing care, within their professional scope, for clients and families with established care plans and predictable outcomes.
- Effectively employs the nursing process to provide superior nursing care for clients and/or caregivers across various situations, integrating current knowledge and research into practice.
- In accordance with the RPN's scope of practice, adheres to the nursing care plan, including discharge strategies. Engages in ongoing evaluation and modification of the care plan through collaboration with clients, the LHIN, physicians, and other multidisciplinary team members.
- Primarily delivers nursing care within the Hamilton Visiting Program, while also extending services to clients in the Integrated Client Care Program as necessary.
- Maintains professional competence and engages in ongoing professional development opportunities, acquiring advanced or specialized nursing skills to meet client care requirements when needed. Conducts self-evaluations in line with the College of Nurses of Ontario Standards of Practice and the SJHC appraisal process.
- Acts as an advocate for clients and caregivers by clarifying information, supporting decision-making, and facilitating effective communication.
- Current RPN registration with the College of Nurses of Ontario, free from restrictions or pending hearings.
- Practical Nursing Diploma from an accredited college program, including evidence of IV medication administration as part of the core curriculum, or an equivalent combination of education and certifications.
- Must possess a wound care certificate from an accredited institution.
- Current registration with the College of Nurses of Ontario.
- Basic Cardiac Life Support (BCLS) certification.
- Ability to function fully within the scope of practice as defined by the College of Nurses of Ontario.
- Experience in conducting comprehensive nursing assessments (e.g., Mini-mental status, depression, pain assessments).
- Proficient in utilizing software for client documentation, word processing, and communication.
- Strong commitment to customer service and professional development.
- Demonstrated good work performance and attendance record.

- Exhibits competencies in Service and Mission Focus, Achievement, Teamwork, Learning and Innovation, Communication, Conflict Resolution, and Resilience.
- Original and current Police Check with Vulnerable Sector Screening.
- Valid Driver's License and proof of $1M automobile liability insurance, with the ability to drive locally and on highways.
- Must be COVID-19 vaccinated.
